The evaluation agency conducted annual inspections and evaluations, randomly selected and reviewed safety evaluation reports, and collected opinions from the municipal, direct management, forestry safety supervision bureaus and some assessed units on the practice behaviors of the evaluation agencies, and reported or reported to the public. In view of the above, overall, since 2011, with the joint efforts of safety supervision agencies and safety assessment agencies in various regions, the safety assessment and supervision mechanism in the province has been further improved, institutional support capabilities have been further enhanced, and business practices have been further standardized to provide technical services. I. Main achievements of safety evaluation work since 2011

Since 2011, the safety supervision departments at all levels have taken strong measures to strengthen the supervision of safety assessment agencies. The province's safety evaluation work has shown a healthy and orderly development trend.

(a) The safety assessment and supervision system has been further improved. Since 2011, the Provincial Safety Supervision Bureau has successively promulgated the "Planning for the Development of Safety Assessment Institutions in Hubei Province" (hereinafter referred to as the "Plots and Point Plan"), the "Regulatory Measures" and the "Examination Rules", which further improved the supervision system of safety assessment agencies in our province. . First, in accordance with the basic thinking of "overall planning, rational layout, total control, and appropriate development," and in line with the reasonable distribution of regions and industries, the number of industries and institutions in the region is balanced, and the institutions and industries in neighboring regions are complementary to each other. The principle of blank industry, put forward the "distribution plan" to solve the province's safety assessment agencies layout is not reasonable, the development is not balanced, the function is not complete, the scope of the business is too concentrated, leading to market competition disorder, evaluation behavior is not standardized and other issues. Second, the newly issued "Supervisory Measures" has clarified the conditions for institutional qualifications, clarified the approval procedures and procedures, distinguished the responsibilities and tasks of the supervision of safety supervision departments at all levels, and solved the problems of over-stretched institutional setup, approval flexibility, difficulty in grasping and institutions. The issue of unclear supervision responsibilities, unclear tasks, and incomplete mechanisms. Regarding the situation of non-standard evaluation behavior, on the basis of arranging laws and regulations focused on assessing penalties for illegal behaviors, it has focused on solving the problem of lack of management and control, such as management gaps and illegal penalties that are inconsistent, and violations of professional standards. The third is to implement “double seven” assessment on safety assessment agencies, that is, seven mandatory standards, one of which is negated; seven management standards are refined into 52 items, and the percentage assessment is implemented, dynamic management is implemented, and the past is changed. (b) The safety assessment mechanism has been further improved. Through the establishment of mechanisms such as the review of safety evaluation reports, interviews with responsible persons of safety assessment agencies, filing of safety assessment agencies in other provinces, and online disclosure of relevant safety assessment reports, the establishment of responsibility awareness among safety assessment agencies has been effectively promoted. Through a series of activities such as comprehensively clearing the basic situation of safety assessment agencies and appraisers and making public announcements, conducting safety appraisal agency supervision and special management of professional behaviors, and training of safety appraisers, effectively promoted the appraisal personnel's professional quality and safety appraisal agency's technology. Improvement of service quality.

(3) The technical support capabilities of safety assessment agencies have been further enhanced. At present, there are 5 Grade A qualification safety assessment agencies and 13 Grade B qualification safety assessment agencies in our province. A total of 411 safety appraisers registered in the aforementioned institutions, including 98 new appraisers and 227 registered safety engineers. The qualifications of the offices, fixed assets, registered capital, general equipment, and evaluation team of the evaluation agencies all remained sound. The improvement of the technical service conditions of the safety assessment agencies, the improvement of the overall quality of the evaluation personnel, and the strengthening of daily supervision and management have led to the standardization of evaluation activities and the improvement of evaluation quality. In 2011, the province’s safety assessment agencies completed a total of 1493 safety assessment reports, pointing out 5941 safety hazards and proposing 9200 corrective measures and recommendations, which made positive contributions to the continuous improvement of safety production in the province.

Second, the main problems

Through the comprehensive inspection and evaluation of safety assessment agencies and the investigation of relevant issues, the safety assessment in our province still has unstable institutional qualifications, internal management needs to be strengthened, heavy economic efficiency, light risk control, heavy reporting volume, light process control, etc. problem.

(a) The process control needs to be strengthened. Some of the safety assessment agencies did not have enough training and learning of process control documents. The evaluation staff, especially the quality process control management personnel, did not master the content of the process control documents well enough. As a result, the process control was not standardized, and the quality of evaluation reports was not high. A small number of safety assessment agencies have neglected management of the evaluation process and it is difficult to play a role in checking and ensuring the quality of evaluation reports. The first is that risk analysis flows into form. Risk analysis should be conducted before the project contract is signed. However, some risk assessment agencies risk analysis after the signing of the project contract, some records are not standardized, and even some individual safety assessment agencies such as the Huanggang Oriental Institute of Safety did not conduct risk analysis. Second, the internal review of the evaluation report is not detailed. A few safety assessment agencies have an evaluation report. The internal audit is performed by members of the project team, and the process control audit is performed by the person in charge of non-process control. Third, the signature of the evaluation report is inconsistent. The signature of the internal audit record of the same evaluation report is inconsistent with the signature of the signature report of the evaluation report; the signature of the same safety assessment agency and the same appraisal staff on the different appraisal reports is inconsistent, and those who did not participate in the appraisal report are blindly signed. The issue of the signature of the appraisal report of Beijing DAF Safety Management Consultant Co., Ltd. is particularly prominent (involving project team member Li Chun and technical director Zhang Zhengyun). Fourth, the report audit record is not detailed. Some security assessment agencies have only signatures, no auditing content, or even empty ones. Some organizations do not have opinions or results on the changes.

(II) The update and implementation of the standard system need to be strengthened. One is that the standard system is not updated in a timely manner. There are widespread problems in the safety assessment agencies that laws, regulations, and technical standards are not updated and that the evaluation reports have expired or incomplete references. In particular, Wuhan Xingye Mine Safety Evaluation Consulting Co., Ltd. pointed out this problem after inspecting and assessing the State Administration of Safety Supervision, and was once again found to be the problem (involving project team members Deng Hang and Zhou Dehong). Second, the internal management system is not perfect. According to relevant regulations, safety assessment agencies should establish 14 management systems. During the inspection, it was found that some safety evaluation agencies did not improve the system as required. Some institutions' systems have not been modified in a timely manner in light of actual conditions. The relevant contents are old, and their pertinence and operability are not strong. Third, the implementation of the standard system is weak. Some established the system but did not implement it seriously. In particular, the “Regulatory Measures” and “Assessment Rules” newly issued by the Provincial Work Safety Supervision Bureau have not been seriously promoted and relevant requirements have not been put in place.

(III) Internal management needs to be strict. First, file management is not standardized. Some institutional archives administrators are not clear about the content and requirements of the archives management, the methods and processes are not familiar with them, and the quality of the business needs to be improved. Some organizations did not establish an index of documents, did not retain drafts for review of evaluation reports, and evaluation reports and image data of individual organizations remained in the hands of evaluation staff and did not conduct archiving management. The second is the incomplete record of individual performance. Some agencies do not know the performance statistics and contents and requirements of reporting. They have not established performance records and statistical mechanisms. There are no detailed records at ordinary times, and the factors are incomplete when statistics are submitted. The third is that the daily management is not in place. The performance did not attach much importance to the maintenance of the qualifications of the organization, and the evaluation personnel’s labor contract and technical expert employment agreement renewal, “four insurances and one payment”, filing of evaluation reports, and performance records were not handled on time.

(d) The evaluation fee is low. According to the “Guiding Opinions on Hubei Province's Safety Evaluation Industry Charges” issued in 2009, all agencies have failed to meet the price requirements for charging guidance, and some institutions’ fees have been significantly lower than the guiding price. Yichang Ante Safety Evaluation Equipment Supervision Co., Ltd. paid a guiding price of 300,000 yuan for the acceptance evaluation project of 60,000 tons/year of silicone monomer project of Hubei Xingrui Chemical Co., Ltd., and the contract amount was only 30,000 yuan.

(5) The quality of the report needs to be improved. First, there are omissions in a few evaluation reports. For example, there is a lack of data support for the evaluation of internal and external distance conformance. The second is that the site survey is not careful. Some evaluation reports do not elaborate on the surrounding environment of the project, and analysis of dangerous and harmful factors is not targeted. Third, the image data is not complete. Some of the evaluation reports lacked on-site image data, in particular the lack of possible geological disasters in the environment and places where there are major hidden dangers, and the company's rectification and other related imagery data. Fourth, the annex to some of the evaluation reports is incomplete. Such as the lack of special equipment inspection reports, lightning detection reports, safety inspection and inspection reports, inspection and design units issued effective drawings.

(6) Qualification conditions are not stable enough. First, personal insurance payment is insufficient. Some institutions fail to complete the insurance coverage, and some fail to pay on schedule. Individual agencies can only provide valid certificates for a few full-time appraisers. They cannot effectively prove the number of full-time appraisers, such as Yichang Ante Safety Evaluation Equipment Supervision Co., Ltd. Only provide valid certification for labor insurance for 5 full-time safety appraisers. According to the “Estimating Rules”, there are 7 criteria for appraisal agency qualifications. The second is the lack of general equipment. Personal protective equipment and computers of individual agencies did not meet the requirements of each set of safety assessment personnel. Third, the number of technical leaders is insufficient. The technical director of an individual agency cannot meet the needs of the professional being declared, and even some individual technical director is not qualified.

Third, the next step requirements

Safety evaluation is the basic work for strengthening safety production, is the pre-measurement method for safety production process control, and is an important basis for the safety supervision agency's legal administration. Safety supervision agencies and safety assessment agencies in various places must pay attention to the overall situation, further enhance the sense of responsibility and urgency in doing a good job of safety evaluation, and take effective measures to solve outstanding problems in the current field of safety evaluation.

(1) Strengthen safety evaluation management. The safety supervision departments at all levels shall, in accordance with the spirit of the relevant documents issued by the State Administration of Work Safety and the Provincial Administration of Work Safety, highlight the evaluation of the key points of behavioral supervision and implement follow-up management of the institutions conducting evaluation activities within their jurisdictions to promptly identify and correct problems. Each safety assessment agency shall revise and perfect the process control documents and internal management system, further clarify job assignments and personnel responsibilities, strengthen the safety risk awareness and responsibility awareness education for safety appraisers, and urge the appraisers to earnestly implement the process control procedures and various systems. It is necessary to strengthen the management of evaluation personnel, supervise and elevate evaluation personnel in compliance with the law, and promote the healthy and orderly development of safety evaluation institutions.

(b) To strictly regulate the evaluation of behavior and process control. All safety assessment agencies and practitioners must establish the correct direction of service. While pursuing economic benefits, they always give top priority to social benefits and the concept of safe production services. We must strictly self-discipline and orderly competition, follow the principles of objectiveness and fairness, honesty and trustworthiness, and open competition, and carry out safety evaluation activities in accordance with laws and regulations. Resolutely put an end to the acts and phenomena where the evaluation activities were not on site, blindly signed by non-project team members, the report provided by non-institutional registered evaluators was arbitrarily stamped, and malicious competition was low-priced. We must firmly establish the concept of adhering to standards and carry out safety evaluation activities, and strive to overcome internal management weaknesses, innovate quantitative management and modular management, and ensure that all aspects of process control, various contents, and all types of personnel are in place to ensure safety assessment. quality.

(3) It is necessary to increase the efforts of the institution itself. Evaluation agencies should establish long-term goals for development and specialization, and intensify the introduction and training of talents. In particular, it is necessary to increase the training of technical backbones, shape the age-structure, education level, professional and technically qualified professional and technical teams, and resolutely eliminate a few The appraisers take classes and most of the appraisers are involved in part-time work. It is necessary to speed up the pace of informatization construction, explore the process of evaluation and control of the report management, increase the scientific and technological content of evaluation activities and internal management, consolidate the foundation, enhance the strength, and enhance the institutional development potential.

(D) to carefully organize self-examination and rectification. Institutions should strengthen the review and promotion of newly issued system standards and management regulations. They must seriously deal with the issues found in this inspection and evaluation work of the State Administration of Work Safety and conduct a thorough clean-up of the management situation and evaluation reports of recent years. In contrast, especially with regard to process control issues, it is necessary to combine self-examination and self-correction with the introduction of new system standards and publicity of management regulations, and seriously carry out rectification to ensure effective resolution of problems.
